Job
Summary:
The PACE Transportation Coordinator is responsible for coordinating and scheduling transportation assignments of PACE participants.
Ensure communication is clear and drivers follow through as directed.
Organize routes and schedule daily transportation for PACE participants to and from the PACE Center and other medical appointments.
Responsibilities:
Attended IDT and POC daily meeting.
Schedule transportation for all PACE participants.
Respond appropriately to needs as identified by the Interdisciplinary Care Team.
Coordinate transportation for PACE participants to ensure that the transportation is on schedule, timelines are met, and are at the highest level of operational efficiency.
Coordinate schedules, cancel schedules, and reschedule trips as needed.
Make adjustments to meet unanticipated changes and communicate those changes to the appropriate affected personnel.
Review and audit daily trips and prepare reports.
Model and promote customer service by developing and maintaining positive relationships with PACE participants, families/caregivers, colleagues and staff throughout the organization.
Maintain adequate records for transportation, and ensure established policies and procedures are communicated and followed.
Respond appropriately to needs as identified by the interdisciplinary Care Team.
Adjust meet unanticipated changes and communicate those changes to the appropriate affected personnel.
Report all issues promptly with possible solutions to Management.
Perform other duties as assigned.
Requirements:
High School Diploma/GED required.
Active driver's license in good standing Meet organizational driving requirements.
